Checklist and Information for Accepted Teams

Tournament registration will be on Friday evening, June 11th from 5:00 to 9:00 at Soccerzone, 7187 Sprinkle Road (adjacent to the Portage Soccer Complex).
TEAMS NEED TO BRING ALL OF THE FOLLOWING ITEMS TO CHECK IN:
   1. Copy of its official league roster (we keep it)
2. Completed PSC tournament roster (click here)
3. Player pass cards
4. Risk management cards for all coaches and managers (maximum 3 per team) on the team sideline
5. Completed waiver of liability form (click here)
6. Medical release forms (example)
7. Guest players require MSYSA guest player form (available at www.msysa.net)
8. Permission to travel form for out of state teams (see your state association)
Please inform your team that no pets are allowed at the Portage Soccer Complex
