Checklist and Information for Accepted Teams
| Tournament registration will be on Friday evening, June 11th from 5:00 to 9:00 at Soccerzone, 7187 Sprinkle Road (adjacent to the Portage Soccer Complex). | |
| TEAMS NEED TO BRING ALL OF THE FOLLOWING ITEMS TO CHECK IN: | |
| 1. Copy of its official league roster (we keep it) | |
| 2. Completed PSC tournament roster (click here) | |
| 3. Player pass cards | |
| 4. Risk management cards for all coaches and managers (maximum 3 per team) on the team sideline | |
| 5. Completed waiver of liability form (click here) | |
| 6. Medical release forms (example) | |
| 7. Guest players require MSYSA guest player form (available at www.msysa.net) | |
| 8. Permission to travel form for out of state teams (see your state association) | |
| Please inform your team that no pets are allowed at the Portage Soccer Complex | |
